Report an Incident or Concern
Bowling Green State University is committed to community well-being and belonging. In turn, it is the responsibility of each person within the University community to take action when an issue or concern arises. Below are various forms to report concerns. Please call the Office of the Dean of Students at 419-372-2843 if you have a question about which form to complete.
If this is an emergency – someone’s health, safety and/or property is at risk, please call 911. These forms are not designed as an emergency response notification process, and staff typically review and respond to reports within one business day.
Student Conduct Report
Use this form to report potential violations of the Code of Student Conduct on or off campus involving students or recognized student organizations. Incidents include but are not limited to: alcohol misuse, drug use, violations of Residence Life or other campus policies, physical assault, organizational misconduct, disruptive behavior, or theft.
Student of Concern Report
Use this form to provide information about a student who is exhibiting behaviors that are of concern in relation to their personal (e.g., food or housing insecurity), physical or emotional well-being.
Hazing Report
Use this form to report hazing information involving students, recognized student organizations, clubs, teams, etc. You can also call the BGSU Hazing Report Line at 419-372-HAZE (4293) to speak with staff from the Office of the Dean of Students 24/7. Any person required by Collin's Law to report knowledge of hazing must immediately report that information to a law enforcement agency in the county in which the victims of hazing reside or in which the hazing is occurring/had occurred.
Title IX and Sexual Harassment Report Concerning Students
Use this form to report incidents involving alleged misconduct by STUDENTS that involve sexual misconduct, sexual harassment, sexual assault, dating violence, domestic violence, stalking or retaliation.
Campus Climate Concerns Report
Use this form to report general campus climate concerns. Campus climate concerns can include actions that impact anyone in the community based on their identity (such as race, color, ethnicity, national origin, sex, gender identity or expression, sexual orientation, disability, age, or religion). Note: To officially report incidents involving allegations of discrimination or harassment based upon perceived or actual protected class, please submit the Harassment/Discrimination Report. To report violations of the BGSU Freedom of Expression Policy and/or Ohio Public Policy on Principles of Free Speech, please submit the Campus Free Speech Complaint Form.

Use this form to report incidents involving alleged misconduct by EMPLOYEES that involve sexual misconduct, sexual harassment, sexual assault, dating violence, domestic violence, stalking, or retaliation.
Harassment/Discrimination Report
Use this form to report incidents involving allegations of discrimination or harassment based upon perceived or actual protected class.
Campus Free Speech Complaint Report
Use this form to report incidents involving potential violations of BGSU Freedom of Expression Policy and/or Ohio Public Policy on Principles of Free Speech.
Use this form to report potential violations of the Code of Academic Conduct.
Religious Accommodation Complaint Form
Use this form to report alleged violations of the BGSU Policy on Student Religious Accommodations.
